For probably 30 years, creatine has occupied the least glamorous shelf in the supplement store. This despite the fact that it is without question one of the most studied substance on the market.

No proprietary “anabolic matrix.” No molecule named something that sounds like a Russian weapons program. Just creatine monohydrate—a cheap white powder that has survived practically every supplement fad we’ve managed to invent.

What Does the Latest Research Say About Creatine for Female Athletes?

Women still don’t use enough of it. Maybe that’s finally changing. A new systematic review and meta-analysis published Sept. 4 examined creatine specifically in women participating in exercise or structured training. The researchers screened more than 13,000 records and ultimately included 34 studies representing 692 female or female-eligible participants; 21 studies provided data suitable for the primary statistical analysis.

The result wasn’t earth-shattering. That’s actually what makes it interesting. Creatine produced a small but statistically significant overall benefit, with positive pooled effects for both performance and body-composition outcomes. The performance effect was about 0.30 standardized units and the body-composition effect about 0.24.

Translation for people who don’t eat statistical analysis with breakfast: It appears to help, but it isn’t magic.

Does Creatine Cause Weight Gain or Bulking in Women?

Fitness marketing has conditioned us to believe a supplement must either transform you into Ronnie Coleman by Thursday or it’s worthless. Real ergogenic aids don’t work that way. They give you an edge. Then you exploit that edge repeatedly through training until little improvements become meaningful ones.

Creatine makes physiological sense because phosphocreatine helps regenerate ATP during brief, high-intensity efforts. That’s why its usefulness tends to show up around repeated hard efforts, strength, power and high-intensity work rather than turning someone into an Olympic marathoner.

The female-specific literature in the new analysis included soccer players, swimmers, volleyball players, dancers, wrestlers and women resistance training in the gym. Some studies showed better strength, sprinting, jumping, repeated-sprint ability or lean-mass measures. Others showed little or no additional benefit.

That’s important because I’m not going to do what supplement companies do and selectively wave the positive studies around while hiding the boring ones under the couch.

The authors themselves rated the certainty of the overall evidence very low, and the performance evidence low, largely because the studies differed substantially in participants, sports, dosing and testing methods. Their conclusion was appropriately restrained: creatine may produce small improvements in selected strength, power, high-intensity performance and lean-mass-related outcomes in women, but the evidence isn’t yet strong enough to make sweeping sport-specific promises.

How Does Creatine Benefit Postmenopausal Women?

Meanwhile, another 2026 meta-analysis looked specifically at postmenopausal women. Seven randomized trials involving 608 participants found a small average increase in lean mass and improved leg-press strength with creatine. The benefits were most evident when at least five grams per day was combined with resistance training.

The Truth About Creatine Effects on Women

There’s a recurring theme here: Creatine plus sitting on your butt isn’t the same thing as creatine plus training.

Women have historically been particularly susceptible to avoiding creatine because of fear of “getting bulky,” gaining weight or retaining water. Yes, creatine can increase intracellular water. That’s part of what creatine does. But confusing hydrated muscle with suddenly waking up built like Arnold is fitness-industry nonsense of the highest order.

A woman isn’t going to accidentally become enormous because she put five grams of creatine in her water bottle any more than I’m going to accidentally become a concert pianist because I stayed in a Holiday Inn Express last night.

The interesting part is that research on women is finally catching up with a supplement we’ve been studying predominantly through male or mixed populations for decades.

And the answer emerging isn’t sexy. Creatine isn’t a female miracle supplement. It isn’t a fat burner. It isn’t a hormone. isn’t going to replace protein, training, sleep or dietary discipline.

It’s something much rarer in the supplement industry: A boring, inexpensive product with enough evidence behind it that we don’t have to make shit up. Plain and simple, creatine monohydrate is the most studied substance our industry has produced. And it works.
